I testify that none is worthy of worship except Allah alone, with no partner, and Muhammad is His servant and messenger. O Allah, make me among those who repent and those who purify themselves. Glory and praise belong to You, O Allah. I testify that none is worthy of worship except You. I ask Your forgiveness and repent to You.

Reference: Sahih Muslim 234a · Jami at-Tirmidhi 55

Source and review note

The testimony is in Muslim 234a. The repentance/purification addition is in Tirmidhi 55, graded weak in the displayed Darussalam edition, with differing assessments of supporting transmission. The final praise needs separate evidence for this context. This is a compilation, not a single prescribed formula.
